Recently the FDA issued a warning regarding the use of the epilepsy and migraine medication Topamax during the first trimester of pregnancy. The Topamax FDA warning stated that there was new data showing evidence of an increased risk for the development of cleft lip and cleft palate (oral clefts) in infants born to women who used Topamax (Topiramate) during pregnancy. The medical community and doctors are recommended to consider alternative drugs that have a lower risk of oral clefts and other birth defects should be considered for these patients. Cleft palate and cleft lip birth defects occur in the first trimester of pregnancy before many women even know they may be pregnant. Cleft lip and cleft palate range from a small notch in the lip to a groove that runs into the roof of the mouth and nose, possibly leading to problems with eating, talking and ear infections.
Based on recent human data that reveal an increased risk for oral clefts, Topamax is being placed in Pregnancy Category D. Pregnancy Category D means there is positive evidence of human fetal risk based on human data but any potential benefits from use of the drug in pregnant women may be acceptable in certain situations despite Topamax cleft palate risks.
